To be considered eligible for the Lifestyle Dentistry Scholarship you must:
A) Be attending college or university no later than September 30th, 2015. Current High School Seniors who will graduate in 2015 are eligible. You can be enrolled in any field of study.
B) Submit answers to the essay question, which can be found on the application page, prior to the June 30th, 2015 deadline.
C) Have a GPA of 2.5 or greater.
D) Be a resident of Canada or the United States
The scholarship winner will be announced roughly six weeks after the deadline and will be posted on this website immediately after. All applications must be received by 11:59pm ET on June 30th, 2015. No late applications will be reviewed.
